If you have changed address, phone number, email address or surname you are able to inform us of this by submitting a Rapid Health form (Questions and admin > Update personal details).

Permission to disclose form 

In order for us to be able to discuss your medical records with another named individual please complete and sign a permission to disclose form and return to us to allow us to update your record.

Physiotherapy self-referral form

If you are over 16 with a musculoskeletal problem such as muscle and joint pain, sports injuries, back pain, neck pain, sprains and strains you can fill out this physiotherapy self-referral form.
