Introduction
Thank you for taking the time to read this section of the website. You will likely have been directed here having received an ‘Annual Review Invite' text message.
Each year we invite in over 2,500 patients for an annual chronic illness review. These are for a range of conditions, including, but not limited to:
- Cardiovascular/heart disease (including high blood pressure (hypertension), heart failure, peripheral arterial disease and atrial fibrillation)
- Stroke and Transient Ischemic Attacks (TIAs which are commonly referred to as ‘mini strokes’)
- Diabetes
- Respiratory diseases (including asthma and COPD)
- Mental health
- Rheumatoid arthritis
How the system works
We will invite eligible patients on the month of their birth. These invites will go out regardless of any reviews you may have had earlier in the year, this is to make sure that people with multiple conditions don't miss out on an invite for review because some of their conditions have already been reviewed.
The Annual Review year runs from April - March. This can mean that if you attended your review quite late in the previous year, and your birthday is in May for example you could be called quicker than a year. We are happy for you to attend again as this will bring you in line with the annual recall.
You can be invited via letter or text message, if you would rather your invites come one way or the other please contact us to let us know. Please note this will affect all future communications via that method not just your Annual Review Invites.
You will receive two invites in the month of your birth, after this the invites will stop. If you should decide you want an annual review later on in the year, this is not a problem. While the link we sent out will no longer work, please feel free to call the surgery to arrange this.
Annual Review invite text message
If you have received a text message from us with an invite link for your annual review it will read something like this:

The SystmOnline link at the bottom of your text will allow you to book directly into one of our HCA (Health Care Assistant) Annual Review appointments. Before you attend your appointment please read the HCA Annual Review letter in the letters section below, and if you have a blood pressure machine at home please fill out the home blood pressure form below and bring it with you.
At your 20 minute Health Care Assistant Annual Review we will do preliminary tests & advice such as a blood pressure check, BMI, blood test, lifestyle advice and if required a diabetic foot check. The healthcare assistant in your appointment will then advise you what happens next.
If you also suffer from Diabetes, Pre-Diabetes, Hypertension, Heart Disease, Atrial Fibrillation or High Cholesterol please make sure you bring a urine sample to your appointment.
The sample should be mid stream from your first urination of the day for best results. Specimen bottles can be picked up from reception if needed.
Diabetic Information
If you have received a text message following on from your annual review with a link for more information please select the relevant link(s) as advised from the options below.
- Research shows the more patients know & understand about their diabetes management, the better they do long-term. Face to face groups have been replaced by online learning. Please visit the healthy living website and check the "I have Type 2 Diabetes" button, complete your details to register, and log-in
- We recommend that patients with Long Term Conditions (LTCs) access psychological support to help them adjust to lifestyle changes required to manage their conditions effectively or to deal with "condition fatigue". Please visit West Sussex Talking Therapies which you can self-refer to.

COPD Invite Message/Questionnaire
If you have COPD you will receive a separate invite specifically related to COPD.
If you are invited by letter you will be asked to book straight in for an appointment by calling the surgery and asked to complete the attached questionnaire.
If you are invited by SMS it will read like this:

Please use the link to fill out the questionnaire, this will then come back to the Admin department. Once you send your questionnaire back a member of the Admin team will call you to arrange an appointment with one of our COPD nurses. Appointments for COPD can be face to face or video so please feel free to ask for whichever you prefer (unless otherwise specified all COPD appointments will be booked as face to face by default). There is a section in the questionnaire if you have any specific questions you want to ask the COPD Nurse/if you want to ask them to speak to you directly.

The letters and forms below are copies of the letters we send out to patients in the mail. If you do not have a printer please feel free to attend the surgery to ask for a printed copy.
